We are currently accepting new patients. We accept most private and government funded insurances. We do not accept any self paying patients.
You can submit a request by filling out the online form or call us at (256)-325-2772. Once we have verified your insurance and insured you are assigned to our practice, you will be contacted by the scheduling department to make a new patient appointment. We must be able to verify your insurance prior to being scheduled for an appointment. We also ask that you sign a release of records either with us or with your previous primary care provider prior to your new patient appointment. Please let the scheduler know if you will need any medication refills or if you are sick and need to be seen soon.

Your First Appointment
Please bring a copy of your driver's license or other form of acceptable identification, insurance card, copy of medical records, and all medications that you are currently taking, both prescribed and over the counter.
